Coverage Criteria and Medical Necessity Rules

inv-01: Cosmetic vs Reconstructive — General

General coverage stance

General Coverage Rule: Cosmetic surgery is excluded from coverage unless required for prompt repair of accidental injury or to improve functioning of a malformed body member; reconstructive surgery for abnormal structures is generally covered when reasonable and necessary.

inv-02: Abdominal Lipectomy/Panniculectomy

Abdominal lipectomy/panniculectomy — coverage when no LCD/LCA exists

Panniculectomy coverage — anatomic and symptomatic criteria: The pannus hangs below the level of the symphysis pubis and causes one or more of the following: inability to walk normally due to pannus size; chronic pain; ulceration created by the abdominal skin fold; or intertrigo dermatitis.

Photographic confirmation may be requested.

Duration and treatment failure: The above symptoms have been present for at least 3 months and are refractory to usual standard medical therapy (e.g., topical antifungals, corticosteroids, antibiotics when appropriate).>= 3 months

Preoperative photographs may be required.

Post–weight-loss stability: If procedure follows significant weight loss, weight must be stable for at least 6 months; if following bariatric surgery, wait at least 18 months after bariatric surgery and only when weight has been stable for the most recent 6 months.stable weight >= 6 months; >=18 months post-bariatric with most recent 6 months stable

inv-03: Dermabrasion

Dermabrasion coverage where no LCD/LCA exists

Dermabrasion covered indications: Dermabrasion is considered reconstructive and reasonable and necessary when treating rhinophyma or when correcting defects resulting from traumatic injury, surgery, or disease.

Dermabrasion for post-acne scarring is considered cosmetic and not covered.

inv-04: Breast Reconstruction Following Mastectomy

Breast reconstruction following mastectomy

Breast reconstruction coverage following medically necessary mastectomy/lumpectomy: Reconstruction of the affected and contralateral unaffected breast following a medically necessary mastectomy or lumpectomy is considered non-cosmetic and covered per Medicare NCD 140.2 and WHCRA guidance; covered services include external prostheses and may include implantable prostheses per referenced policies.

Refer to NCD 140.2 and UnitedHealthcare Breast Reconstruction policy for implant and ADM guidance.

Not covered when: Reconstructive services requested solely for cosmetic symmetry in members who have not had a reasonable and necessary mastectomy or lumpectomy are not covered.

Program payment may not be made for breast reconstruction for cosmetic reasons.

inv-05: Lipectomy reconstructive coverage

Covered when ALL of the following are met

Anatomic/symptomatic indication: Lipectomy of thigh/leg/hip/buttock/arm/forearm/hand/submental fat pad is performed to alleviate complicating factors such as ulceration created by a skin fold OR intertrigo dermatitis.

Duration and refractory requirement: The above symptoms have been present for at least three months and are refractory to usual standard medical therapy.>= 3 months

Document medical therapy tried and response.

inv-06: Punch graft hair transplant coverage

Covered when ANY of the following are met

Punch graft hair transplant indications: Performed for eyebrow(s) or symmetric hairline replacement following burn injury, trauma, or tumor removal.

Document indication (burn, trauma, tumor) in the record.

inv-07: General Coverage Rules and References

Coverage and review guidance

LCD/LCA compliance: Where Medicare LCDs/LCAs exist for a listed procedure (for example, suction assisted lipectomy of the trunk), compliance with those local policies is required.

Cosmetic vs reconstructive determination: Procedures such as toe polydactyly reconstruction and unlisted craniofacial/maxillofacial procedures may be cosmetic; individual review is required to determine classification under CMS definitions.

inv-08: Concurrent panniculectomy with ventral hernia repair

Evidence summary and implications

Concurrent panniculectomy evidence summary: A 2024 systematic review and meta-analysis (Rasador et al.) found concurrent panniculectomy with open ventral hernia repair was associated with lower hernia recurrence (RR 0.74) but higher rates of surgical site infection (RR 1.31), surgical site occurrence (RR 1.49), skin necrosis (RR 2.94), and reoperation (RR 1.73), and a longer hospital stay (mean +0.90 days); studies were predominantly retrospective with heterogeneity and limitations.

Careful risk–benefit consideration recommended when planning concurrent procedures.

inv-09: Outcomes: functional panniculectomy

Retrospective cohort findings

Outcomes: functional panniculectomy vs cosmetic abdominoplasty: Retrospective data (Kalmar et al. 2022) show functional panniculectomy patients have higher overall adverse events, medical and surgical complications, readmission, and reoperation compared with cosmetic abdominoplasty; increased infections, dehiscence, bleeding requiring transfusion, and sepsis were reported.

Results limited by retrospective study design and higher comorbidity burden in functional panniculectomy patients.

inv-10: Functional panniculectomy coverage criteria

Covered when ALL of the following are met (functional panniculectomy indication as described in specialty guidance):

Functional indication requirements: 1) Documented functional impairment or skin disease (e.g., recurrent intertrigo or panniculitis) attributable to the pannus; 2) Failure of at least 3 months of appropriate medical therapy for skin conditions when applicable (topical antifungals, corticosteroids, antibiotics); 3) Pannus extends below the level of the pubis with photographic confirmation; 4) Weight stability: at least 6 months stable weight if non-bariatric weight loss, or at least 18 months post-bariatric surgery with the most recent 6 months stable.

inv-11: Concurrent procedure documentation and review

When considering panniculectomy concurrently with another major abdominal procedure, consider risks and benefits; coverage may be subject to additional review if ALL of the following apply:

Concurrent-procedure risk–benefit documentation: 1) The indication for the concurrent primary procedure is documented; 2) The rationale for simultaneous panniculectomy with explicit discussion of increased wound morbidity and reoperation risk is documented; 3) Patient comorbidities and BMI are addressed and perioperative optimization is documented; 4) A perioperative antibiotic plan and postoperative monitoring plan are documented.

Evidence shows some studies report reduced hernia recurrence but increased wound morbidity and longer LOS with concurrent procedures; prior authorization may be required.

inv-12: Evidence-based management summaries

Clinical conclusions and management recommendations drawn from cited reviews and studies:

Craniofacial fibrous dysplasia management: Surgical treatment is indicated for facial asymmetry, functional impairment, nerve damage, or lesion progression; radical resection shows lower recurrence but must be weighed against morbidity, with debulking/contouring as alternatives when appropriate.

Avoid preventative optic nerve decompression unless visual acuity is reduced; monitor asymptomatic disease; bisphosphonates for pain management.

Orbital floor reconstruction material selection: Meta-analysis suggests P(L/DL)LA, porous polyethylene, and titanium mesh are associated with less postoperative diplopia and enophthalmos than autologous bone grafts, though evidence is insufficient for definitive endorsement of a single material.

Further comparative studies are needed.

Midface surgery ocular outcomes: Midface corrective surgeries can significantly affect ocular outcomes; strabismus may worsen while binocular vision can improve; ophthalmologic assessment is important pre- and post-operatively.

Studies are limited by retrospective designs and small samples.

Cosmetic surgery is excluded from coverage. Per the Medicare definition referenced in this policy, cosmetic surgery is surgery performed to reshape normal structures of the body to improve appearance and self‑esteem, and expenses incurred in connection with such surgery are not covered except when required for the prompt repair of accidental injury or to improve the functioning of a malformed body member. When determining coverage, refer to applicable CMS NCDs/LCDs and the Medicare Benefit Policy Manual, Chapter 16 §120.

Examples of procedures that this policy identifies as considered cosmetic (not reasonable and necessary) where no applicable LCD/LCA exists include: abrasion of skin lesions (e.g., CPT 15786/15787), chemical exfoliation for acne, electrolysis hair removal, subcutaneous injection of filling material (e.g., CPT 11950–11954), rhytidectomy procedures (e.g., CPT 15824–15829), suction‑assisted lipectomy of head and neck (CPT 15876), and injection of deoxycholic acid (HCPCS J0591). The CPT/HCPCS code listing in this policy is informational; inclusion in the list denotes that the procedure is considered cosmetic in the absence of a local coverage determination.

Panniculectomy is not reasonable and necessary when performed primarily to improve appearance (i.e., cosmetic abdominoplasty) or to repair abdominal wall laxity/diastasis recti. For functional indications, the pannus must hang below the symphysis pubis with documented symptoms (e.g., inability to walk normally, chronic pain, ulceration, or intertrigo) present for at least 3 months and refractory to standard medical therapy. Preoperative photographs and evidence of weight stability (≥ 6 months stable weight after non‑bariatric loss; ≥ 18 months after bariatric surgery with most recent 6 months stable) may be required to substantiate a functional indication.

Injection of deoxycholic acid is considered cosmetic and not reasonable and necessary in the absence of an applicable LCD/LCA. Claims for deoxycholic acid injections may be denied on that basis; providers should follow any relevant local coverage guidance where it exists.

Otoplasty is considered cosmetic and not reasonable and necessary in states/territories without an applicable LCD/LCA. Where a relevant LCD/LCA exists, providers must follow that local guidance; otherwise coverage is excluded under the Medicare definition of cosmetic surgery.

Rhytidectomy (facelift) procedures are considered cosmetic and not reasonable and necessary in states/territories without an applicable LCD/LCA. When local Medicare guidance exists, follow the LCD/LCA; absent such guidance, these procedures are excluded from coverage as cosmetic.

Subcutaneous injection of filling material (e.g., collagen, soft‑tissue fillers) is considered cosmetic and not reasonable and necessary in states/territories without an applicable LCD/LCA. Providers should consult any applicable LCD/LCA; otherwise claims for these injections may be denied.

Suction‑assisted lipectomy of the head and neck (liposuction of head/neck regions) is considered cosmetic and not reasonable and necessary in states/territories without an applicable LCD/LCA. Follow local CMS guidance where present; absent such guidance, do not expect coverage for procedures performed solely for aesthetic contouring of these areas.
