The World Health Organization's 2023 guideline for nonsurgical management of chronic primary low back pain states that traction should not be used as part of routine care for adults with chronic primary low back pain. This recommendation is conditional and driven by very low certainty evidence and a determination that harms may outweigh benefits.

The policy rationale is that published, peer‑reviewed clinical evidence for home traction therapy is insufficient, conflicting, or poor in quality to demonstrate a net benefit for low back or neck disorders with or without radiculopathy. Based on that evidence base, home traction therapy is considered unproven and not medically necessary for these indications.

Systematic reviews and guideline statements conclude that neither intermittent nor continuous traction has demonstrated clinically meaningful benefit for subacute or chronic low back pain. The ACP conclusions (2017 and earlier reviews) report insufficient evidence to support traction alone or in combination with other therapies, and older ACP/American Pain Society guidance (2007) found traction was not proven effective for chronic low back pain. The North American Spine Society guidance (2020) likewise indicates traction provides no clinically significant improvement in pain or function for patients with subacute or chronic low back pain.

Home traction is not recommended as first-line therapy. Clinical guidelines (NICE 2016, NASS 2020, ACP 2017, WHO 2023) advise against offering traction for low back pain with or without sciatica and recommend alternative noninvasive treatments. Before considering traction, document that guideline-recommended alternatives were attempted and the clinical rationale for traction as a next step.

Offer/trial conservative therapies first: structured exercise programs, supervised physical therapy, and patient education/self-management.
Consider manual therapy (spinal manipulation, mobilization, or soft tissue techniques) only as part of a package that includes exercise and, when appropriate, psychological therapies.
Document duration, frequency, and patient response to alternatives (e.g., dates of PT, type of exercise program, adherence, and objective functional changes).
If traction is considered despite guideline recommendations, provide explicit clinical justification referencing failure of trials of alternatives and shared decision-making with the patient.
