The American Academy of Neurology (AAN) 2018 practice guideline (reaffirmed 2021) does not endorse or specifically address the use of stimulation as a treatment for patients with a prolonged disorder of consciousness. The guideline advises counseling families about the limited evidence for such interventions and notes that there are no established therapies for children with prolonged disorders of consciousness.

Coma Stimulation (also called coma arousal, coma responsiveness, multisensory stimulation, or coma care programs) is considered unproven and not medically necessary due to insufficient evidence of efficacy and the absence of established protocols or definitive patient selection criteria. The policy explicitly states that available literature does not support coma stimulation as an effective therapy for any disorder of consciousness.

Evidence for cognitive training and Cognitive Rehabilitation in individuals with mild to moderate Alzheimer disease or vascular dementia is limited and inconsistent. A Cochrane review (Bahar-Fuchs, 2013) found no clear evidence that cognitive training improves cognition, mood, or activities of daily living, noting only a single trial of Cognitive Rehabilitation with preliminary signals for ADL benefit and a recommendation for more high-quality trials.
