This policy governs determination of whether medications are considered self-administered and therefore excluded from the medical benefit for Individual Exchange benefit plans (except MA, NV, NY). It affects providers and payers deciding medical vs drug benefit coverage.

Coverage Criteria — Self‑Administered Medication Determination

Self-administered medication determination (exclusion from medical benefit)

Medication is considered self-administered (and thus excluded from the medical benefit) when ALL of the following are met:

Self-administered determination: Medication is not typically administered or directly supervised by a qualified provider or licensed/certified health professional in an outpatient setting; and does not require continuous or periodic monitoring immediately before, during, or after administration by a qualified provider or licensed/certified health professional in an outpatient setting; and route of administration is consistent with self-administration (e.g., oral, inhaled, intranasal, topical, rectal, subcutaneous or self-injectable intramuscular injections); and dosage form is consistent with self-administration (e.g., prefilled syringe, auto-injector, tablet, capsule, suppository, nasal spray, metered dose inhaler, nebulized solution); and acuity of condition (e.g., chronic disease) supports self-administration; and frequency of administration supports self-administration; and the medication is not specifically allowed under the medical benefit; and standards of medical practice allow self-administration (e.g., self-infused hemophilia factor); and evaluation of established medical literature or compendia (including FDA prescribing information, manufacturer medical literature, peer-reviewed literature, evidence-based practice guidelines, self-administration utilization statistics, and compendia such as IBM Micromedex DRUGDEX and Clinical Pharmacology) has been performed.

All criteria listed must be considered together to determine self-administered status.

Clinician-administered dosage formulations of drugs that appear on the Self-Administered Medications List — for example, an intravenous infusion preparation of a drug otherwise designated as self-administered — may be covered under the medical benefit when delivered in a clinician setting. Refer to the Self-Administered Medications List for the applicable HCPCS or revenue coding (for example, revenue code 0637 is cited as the example revenue code for self-administered drugs).

Medications that meet the policy’s criteria for being self-administered are excluded from the standard medical benefit and therefore are not covered under the medical benefit.
