Vivitrol (naltrexone powder for suspension for injection, extended‑release) is a commercially manufactured product and is explicitly excluded from this policy’s coverage restrictions.

Pharmacy‑compounded subcutaneous naltrexone implants (commonly described as compounded implantable drug pellets) are not FDA approved. The published literature assessing these compounded naltrexone implants is limited and of low to very low quality; systematic reviews and national reviews have concluded that available studies are small, methodologically weak, and do not provide sufficient evidence to determine safety or efficacy.
