Adult HR+ HER2- PIK3CA-negative recurrent unresectable or stage IV: Used in combination with an aromatase inhibitor OR fulvestrant AND provider attests the patient has a contraindication, history of intolerance, or is not an appropriate candidate to both Kisqali (ribociclib) and Verzenio (abemaciclib) (reason documented)

Authorization period: 12 months

Continuation of therapy: As continuation of therapy AND patient has not been established on Ibrance via manufacturer-supplied samples or Pfizer Oncology Together assistance (if they were, they must meet initial authorization criteria)

Providers must document one of the following for Ibrance (palbociclib) prior authorization requests: - Prior-step justification: Document that the member tried and failed, was intolerant to, or had a contraindication to both ribociclib (Kisqali) and abemaciclib (Verzenio). Include dates, doses, treatment duration, and specific adverse events or contraindicating conditions. - Provider attestation of prior-step failure or contraindication: A signed attestation from the prescriber stating the member’s inability to use both alternative CDK4/6 inhibitors (Kisqali and Verzenio) due to documented failure, intolerance, or contraindication. The attestation must state the clinical reason(s). - Documentation regarding prior samples/assistance: For members claiming established use of Ibrance based on samples or patient assistance, provide documentation of the manufacturer-supplied sample, sample card redemption, Pfizer Oncology Together assistance, or a pharmacy 30-day free trial. If such assistance was provided, the member is considered new-to-therapy and must meet initial authorization criteria for Ibrance as if newly started. - Sample-established patients: If the patient was established on Ibrance solely through receipt of no-cost samples in the prescriber’s office or any patient assistance program, initial authorization requirements apply and the provider must submit the documentation described above rather than rely on continuation criteria. Failure to provide the required documentation or attestation may result in denial of the request.
