Management of ESA hyporesponsiveness: Evaluate patients with initial or acquired ESA hyporesponsiveness and treat specific causes; if hyporesponsiveness persists despite correcting causes, individualize therapy considering Hb decline, ESA doses required, and alternatives such as transfusion.

PRCA evaluation: Investigate for antibody-mediated PRCA when a patient receiving ESA therapy for more than 8 weeks develops: (1) sudden rapid decrease in Hb (0.5–1.0 g/dL per week) or requirement of transfusions at ~1–2 per week; (2) normal platelet and white cell counts; (3) absolute reticulocyte count less than 10,000/µL. If suspected, stop ESA therapy and pursue antibody testing; peginesatide is recommended to treat antibody-mediated PRCA per guidance.

Adjuvant therapy recommendations

Adjuvant avoidance: Do not use androgens as an adjuvant to ESA treatment; suggest not using adjuvants including vitamin C, vitamin D, vitamin E, folic acid, L-carnitine, and pentoxifylline.

This policy excludes several scenarios from ESA coverage. ESAs are considered unproven for patients receiving curative chemotherapy, for patients receiving hormonal agents, biologic products, or radiotherapy alone unless they also receive concomitant myelosuppressive chemotherapy, and for patients who require immediate correction of anemia as a substitute for red blood cell (RBC) transfusion. ESAs are also unproven for perioperative use in cardiac or vascular surgery or for patients who will donate autologous blood pre-operatively. In oncology, ESAs are unproven when the anticipated outcome is cure or when anemia can be managed by transfusion; see NCCN guidance for details.

Trials and meta-analyses in oncology have shown that using ESAs to reach higher hemoglobin targets (for example, attempted maintenance in the 12–14 g/dL range) is associated with harm, including increased mortality and higher rates of thromboembolic events. Clinical guidelines therefore recommend restricting ESA use in cancer to settings of myelosuppressive chemotherapy where benefits and risks are carefully weighed, and require documented counseling of patients on ESA risks versus transfusion prior to therapy.

ESAs are not indicated for patients with cancer who are not receiving myelosuppressive therapy or who are receiving non-myelosuppressive therapy. The NCCN guidance and this policy reserve ESA therapy for anemia related to myelosuppressive chemotherapy (and select hematologic indications), and discourage use when a treatable cause of anemia is identified or when patients are receiving only hormonal, biologic, or radiotherapy without myelosuppressive agents.

FDA approval and product labeling are provided here for informational context only. FDA approval alone is not a basis for coverage; coverage determinations under this policy depend on applicable federal, state, and contractual benefit requirements and the clinical criteria specified in this document.

Certain uses of ESAs are described in the policy as unproven or not medically necessary—for example initiating or continuing ESA when hematocrit is at or above specified cutoffs (e.g., dialysis patients with Hct ≥ 33% or non‑dialysis CKD patients with Hct ≥ 30%). The KDIGO-based guidance in this policy also emphasizes avoiding intentional maintenance of hemoglobin above 11.5 g/dL and never intentionally increasing Hb above 13.0 g/dL in adults; decisions above recommended thresholds must be individualized and documented.

Routine administration of epoetin for some neonatal or other off‑label indications is not recommended because benefits are limited and evidence is insufficient. Separately, the FDA‑label warnings for epoetin and darbepoetin include reports of pure red cell aplasia (PRCA); if antibody‑mediated anemia is suspected, ESAs should be withheld and manufacturers contacted per prescribing information.

When initiating or continuing ESA therapy the policy highlights KDIGO-derived targets and monitoring: consider initiation in CKD non‑dialysis patients when Hb < 10.0 g/dL with individualized decision-making; in dialysis (CKD 5D) start ESA to avoid Hb falling below 9.0 g/dL (start when Hb is between 9.0–10.0 g/dL). During maintenance, do not use ESAs to maintain Hb above 11.5 g/dL and do not intentionally raise Hb above 13.0 g/dL. Monitoring frequency and clinical rationale for departures from these thresholds must be documented.

The guideline work group recommends against using androgens as adjuvants to ESA therapy and suggests avoiding other adjuvants (e.g., vitamin C, vitamin D, vitamin E, folic acid, L‑carnitine, pentoxifylline). These agents are not supported as routine adjuncts to ESA treatment and should not be used in place of evaluating and addressing treatable causes of poor ESA response.
