A specialty drug may be determined medically necessary for the requested off-label or unproven indication when ALL of the criteria are met:

The requested drug has not been excluded from coverage by UnitedHealthcare due to lack of efficacy, clinical benefit, or administrative program (e.g., exclusion at launch, plan document)
