Multiplex molecular testing for gastrointestinal pathogens offers improved analytical sensitivity and substantially faster turnaround times compared with traditional culture and non-molecular methods, and can detect a broad range of bacterial, viral, and protozoal agents simultaneously. Systematic reviews and multiple primary studies report higher pathogen detection yields and shorter time-to-result, supporting use when rapid organism-level results may alter clinical management or infection control.

Noted limitations include the inability of NAATs to provide antimicrobial susceptibility or full organism subtyping (i.e., culture is still needed for susceptibility and public health surveillance), the potential to detect nucleic acid from nonviable organisms (so results are not tests of cure), variable panel composition with some targets of uncertain clinical significance (e.g., EAEC, EPEC, certain viruses), and inconsistent clinical outcome evidence. For Salmonella detection, some comparative studies found enrichment culture to be more sensitive than PCR, and parallel culture is recommended when PCR is used.
