Initial Therapy

Covered when ALL of the following are met for initial therapy:

Diagnosis of moderate to severe systemic lupus erythematosus, without severe active central nervous system lupus or severe active lupus nephritis.

Currently receiving at least one standard of care treatment for active systemic lupus erythematosus (e.g., antimalarials, corticosteroids, or immunosuppressants) that is not a biologic.

History of failure, contraindication, or intolerance to Benlysta intravenous (IV) or subcutaneous (SQ).

Patient is not receiving Saphnelo in combination with a biologic agent or Benlysta.

Saphnelo is dosed according to U.S. Food and Drug Administration (FDA) labeled dosing for SLE.

Prescribed by or in consultation with a rheumatologist.

Initial authorization is for no more than 12 months.

Continuation Therapy

Covered when ALL of the following are met for continuation of therapy:

Patient has previously received Saphnelo injection for intravenous infusion.

Documentation of positive clinical response.

Patient is without severe active central nervous system lupus or severe active lupus nephritis.

Currently receiving at least one standard of care treatment for active systemic lupus erythematosus (e.g., antimalarials, corticosteroids, or immunosuppressants) that is not a biologic.

Patient is not receiving Saphnelo in combination with a biologic agent or Benlysta.

Saphnelo is dosed according to U.S. FDA labeled dosing for SLE.

Saphnelo is not indicated for patients with severe active lupus nephritis or severe active central nervous system (CNS) lupus; the efficacy of Saphnelo has not been evaluated in these populations and use is not recommended. In addition, Saphnelo should not be used in combination with other biologic therapies. Requests for coverage that propose treatment in these settings will not meet the coverage criteria and are subject to denial.

Saphnelo is considered unproven and not medically necessary for the following situations: severe active lupus nephritis, severe active CNS lupus, and concomitant use with other biologic agents. Coverage is not supported for these uses because clinical efficacy has not been demonstrated and combinations with other biologics are not recommended.

In addition, there must be history of failure, contraindication, or intolerance to Benlysta (belimumab) IV or SQ for initial approval per policy criteria.

Required documentation to support approval includes: confirmed diagnosis of moderate to severe systemic lupus erythematosus (without severe active CNS lupus or severe active lupus nephritis), current use of at least one standard non-biologic SLE therapy, documentation of trial/failure/contraindication/intolerance to Benlysta (for initial therapy), dosing consistent with FDA‑labeled Saphnelo dosing, and that the prescriber is or consulted with a rheumatologist. For continuation requests include prior administration of Saphnelo and documentation of positive clinical response.
