Treatment of acute HAE attacks — Berinert, Ruconest, Kalbitor (Initial and Continuation)

Covered when ALL of the following are met

Diagnosis of hereditary angioedema (HAE) as confirmed by either: (A) C1 inhibitor (C1-INH) deficiency/dysfunction (Type I or II) demonstrated by C1-INH antigenic level or functional level below the lower limit of normal; OR (B) normal C1-INH with either a confirmed pathogenic variant in an implicated gene (e.g., factor XII, angiopoietin-1, plasminogen-1, kininogen-1, myoferlin, HS3ST6) OR recurrent angioedema refractory to high-dose antihistamines with a compatible family history or de novo mutation (HAE-unknown).
