Inpatient hospitalization is medically necessary when ALL of the following are met:

Inpatient required for listed multi-day/complex regimens: Inpatient stay is medically necessary for regimens that require inpatient monitoring or complex administration over multiple days, including EPOCH/DA-EPOCH/R-EPOCH (continuous infusions days 1–4 with cyclophosphamide day 5), ESHAP/R-ESHAP, interleukin‑2 infusion, high‑dose ifosfamide (>1 g/m2/day), high‑dose methotrexate (>500 mg/m2) with leucovorin rescue, and Hyper‑CVAD cycles as specified.

Factors contributing to need for inpatient stay: Factors include coordination of multiple infusions or multiple drugs over ~96 hours, need for continuous cardiac monitoring (e.g., interleukin‑2), close monitoring of serum electrolytes, creatinine, bilirubin, and urine output, vasopressor support, monitoring of methotrexate serum levels, bladder irrigation (cyclophosphamide), and proximity to intensive care unit.

Outpatient/observation possible: Selected patients may receive certain multi‑day regimens (for example, DA‑EPOCH, ambulatory ifosfamide regimens, or high‑dose cytarabine consolidation) in an outpatient or observation setting when there is established capability for ambulatory infusion (e.g., elastomeric pumps), scheduled monitoring (frequent labs and clinic visits), adherence to written protocols, and the ability to rapidly manage complications such as febrile neutropenia or pump malfunction.

Feasibility and safety are supported by retrospective cohort studies and case series in selected populations; close follow‑up and protocolized care are required.
