Medically Necessary Inpatient/Observation Indications

Inpatient hospitalization is considered medically necessary for chemotherapy when ALL of the following regimen- or condition-specific criteria are met:

Multi-day continuous infusion regimens requiring coordination over multiple days: Regimens that require inpatient monitoring or complex administration over multiple days such as EPOCH/DA-EPOCH/R-EPOCH (continuous infusions over days 1–4 with coordination of multiple infusions over up to 96 hours), ESHAP/R-ESHAP (continuous infusions days 1–4, including high-dose cytarabine), Hyper-CVAD (cycles with high-dose methotrexate/cytarabine components), interleukin-2 infusions (multiple doses over days with need for continuous cardiac monitoring), high-dose ifosfamide (>1 g/m2/day), and high-dose methotrexate with leucovorin rescue (>500 mg/m2).continuous infusions over up to 96 hours or specified high-dose thresholds
