Summary of clinical coverage-relevant findings and guideline positions supporting use in defined circumstances

IPC for immobile post-stroke patients: Use of IPC significantly reduces risk of DVT and improved survival in immobile patients after stroke as demonstrated by CLOTS 3 and supporting meta-analyses.

CLOTS 3: absolute DVT risk reduction 3.6%; mean IPC duration ~11 days; improved 6-month survival (HR 0.86).

IPC as alternative when pharmacologic prophylaxis contraindicated: Guidelines (ASH, European) suggest using IPC for patients at high bleeding risk or when pharmacologic prophylaxis is not used; mechanical prophylaxis is favored over no prophylaxis in such cases.

ASH conditional recommendations (very low certainty); European guidance recommends IPC in high bleeding risk.

IPC combined with pharmacologic prophylaxis in high-risk patients: For patients at very high VTE risk, combined IPC plus pharmacologic prophylaxis may further reduce DVT; some guideline evidence supports combined prophylaxis over pharmacologic alone in selected high-risk patients.

Mechanical prophylaxis (including antiembolism stockings and pneumatic compression devices) is contraindicated in patients with the conditions listed by NICE: suspected or proven peripheral arterial disease; peripheral arterial bypass grafting; peripheral neuropathy or other sensory impairment; any local condition where mechanical prophylaxis may cause damage (for example, fragile skin, dermatitis, gangrene, or recent skin graft); severe leg edema; and major limb deformity or unusual leg size/shape that prevents a correct fit.

Advanced intermittent pneumatic compression devices (APCDs) intended for the head, face, or neck (for example, Flexitouch models for head and neck lymphedema) are designated not medically necessary due to insufficient and low-quality evidence establishing efficacy, clinical value, or safety for lymphedema of the head, face, or neck.

Comparative evidence does not consistently demonstrate that IPC used alone is superior to pharmacologic chemoprophylaxis. Systematic reviews indicate IPC may be comparable to anticoagulation for major outcomes in some surgical and neurological populations, and sensitivity analyses show that IPC alone is neither clearly superior nor inferior to chemoprophylaxis; benefits are context-dependent and may be greater when IPC is combined with pharmacologic prophylaxis in selected high-risk patients.

Immobile post-stroke patients: Immobilized patients after acute stroke (within days of event, unable to ambulate independently) may be appropriate candidates for IPC to reduce DVT risk.

CLOTS 3 inclusion: admission within 3 days of stroke; IPC applied until discharge/walking independently (up to 30 days); mean use ~11 days.

Patients with contraindication to pharmacologic prophylaxis: Patients at high bleeding risk or with contraindication to anticoagulation may be candidates for mechanical prophylaxis (IPC) per guideline recommendations.

ASH conditional recommendations and European guidance support IPC when pharmacologic prophylaxis is contraindicated.

Requests to provide mechanical prophylaxis for patients who have any of the NICE-listed contraindications (for example, suspected or proven peripheral arterial disease, peripheral arterial bypass grafting, peripheral neuropathy or sensory impairment, fragile skin or recent skin graft, severe leg edema, or major limb deformity preventing correct fit) should be denied as not covered because mechanical prophylaxis is contraindicated in these conditions.
