Assessment and use of salivary diagnostics / flow measurement

Initial evaluation: A detailed health history is performed to identify underlying causes of dry mouth (xerostomia).

ADA recommends comprehensive evaluation to facilitate detection of underlying causes and management.

Measurement documented: Unstimulated and stimulated whole salivary flow rates are measured and recorded as a baseline when feasible.

Sialometry (quantitation of unstimulated and stimulated whole saliva) is the preferred clinical method per systematic reviews; results should be documented to rate subsequent complaints and treatment response.

Clinical indication for measurement: Assessment of salivary flow by measurement is indicated for the presence of systemic disease known to cause xerostomia, for polypharmacy, for prior radiation therapy to the head and neck, or when clinical complaints of dry mouth affect oral health.

There are no FDA-cleared salivary diagnostic tests for evaluating the risk of periodontal disease, dental caries, or head and neck cancer. This policy notes that while salivary biomarkers are an active and promising area of research, no salivary assays with FDA clearance currently exist to support routine diagnostic use for these oral conditions.

The American Dental Association states that presently there are no FDA-approved salivary diagnostic tests for evaluating risk of periodontal disease, dental caries, or head and neck cancer and emphasizes the need for large-scale, multicenter clinical trials, independent validation, identification of disease-specific markers, and standardization of collection and storage before clinical adoption.

The current evidence for salivary biomarkers is described as promising but limited. Systematic reviews and meta-analyses have identified candidate markers (for example, MIP-1α, IL‑1β, IL‑6, IL‑8 and other salivary cytokines) with variable diagnostic accuracy, but study sizes, heterogeneity, and methodological limitations preclude broad clinical implementation. Authors and the ADA consistently call for larger, multicenter studies, independent validation, and standardized methods before recommending salivary diagnostics for routine clinical use.

Routine salivary diagnostic testing without validated, disease-specific biomarkers and independent evidence of clinical utility is unsupported. Existing reviews report variability across studies, limited sample sizes, and lack of methodological standardization (including collection and assay procedures), and therefore routine screening or diagnostic use cannot be endorsed until robust validation is available.

Background

Saliva is a complex biofluid composed primarily of water, mucus, proteins (including enzymes such as amylase), minerals, and host- and microbe-derived molecules. It contributes to oral lubrication, protection of teeth from bacterial acids, enamel remineralization, and initiation of digestion. Objective hyposalivation—reduced salivary gland function—raises risk for dental disease and can result from systemic disease, polypharmacy, or head and neck radiation. Saliva testing (bacterial profiles, biomarker panels, and measurement of quantity/quality) is investigational and may provide additional diagnostic information, but clinical relevance depends on validated assays and standardized measurement methods.
