Coverage Criteria for Alveoloplasty and Vestibuloplasty

Alveoloplasty Indications: Alveoloplasty is indicated for bone recontouring and smoothing as part of the tooth extraction process; as a standalone procedure prior to fixed or removable prosthetic construction; to provide stability for implant placement; and for debulking procedures for pathologic conditions of the bone.

Vestibuloplasty Indications: Vestibuloplasty is indicated for ridge extension or lowering/altering submucous displacing attachments prior to prosthetic construction; to complement and complete osseous reconstruction of edentulous bone; to correct inadequate or inappropriate soft tissue drape after prior resection when prosthetic restoration requires improvement; and for overall stability of a dental implant and maintenance of peri-implant bone health.

Not Indicated / Contraindications

Not indicated (coverage contraindications) when ANY of the following are present:

Contraindications: When removing bone would harm vital structures; diminished volume or atypical architecture of bone; prior radiation therapy to the head and neck; unmanaged medical conditions (including but not limited to metabolic, cardiovascular, autoimmune/inflammatory, or genetic conditions affecting collagen synthesis); use of medications that negatively affect healing (including but not limited to immunosuppressive agents, corticosteroids, anticoagulants, NSAIDs, and nicotine); or minimal alveolar ridge height (for vestibuloplasty).

Procedures are not indicated for individuals who have undergone radiation therapy to the head and neck; when removing bone would risk damage to vital structures; or when there is diminished volume or atypical bone architecture that precludes safe or effective recontouring. These contraindications also include patients with unmanaged medical conditions (including but not limited to metabolic, cardiovascular, autoimmune/inflammatory, or genetic conditions affecting collagen synthesis) and individuals taking medications that negatively affect healing (including but not limited to immunosuppressive agents, corticosteroids, anticoagulants, NSAIDs, and nicotine). Additionally, vestibuloplasty is not indicated when minimal alveolar ridge height is present.

Alveoloplasty or vestibuloplasty is considered not medically necessary when any of the contraindications above are present. Examples include prior radiation therapy to the head and neck, unmanaged comorbidities that impair healing, use of medications that negatively affect the healing response, situations where removing bone would harm vital structures, or when inadequate alveolar ridge height or atypical bone architecture means the procedure will not provide clinical benefit.

Background

Alveoloplasty is a surgical procedure to recontour and smooth the alveolar bone, commonly performed at the time of or following extractions to create an optimal foundation for fixed or removable prostheses or for implant placement.
