This policy defines UnitedHealthcare Commercial and Individual Exchange coverage for catheter ablation to treat atrial fibrillation. It applies to UnitedHealthcare Commercial and Individual Exchange benefit plans and does not apply to members aged < 18 years or to arrhythmias other than atrial fibrillation. Coverage where proven or medically necessary is determined by meeting InterQual CP clinical criteria for electrophysiology procedures and catheter ablation.
