Non-Medical Necessity Plans: Cinryze is proven for the treatment of hereditary angioedema (HAE) when BOTH of the following are met: used for prophylaxis against HAE attacks; and not used in combination with other products indicated for prophylaxis against HAE attacks (e.g., Haegarda, Takhzyro).

Berinert — IMPACT trials and extensions: IMPACT‑1 (randomized controlled) demonstrated that 20 U/kg improved median time to onset of symptom relief vs placebo (0.5 vs 1.5 hours; p=0.0025) and IMPACT‑2 and long‑term analyses (1085 attacks in 57 patients) showed consistent responses with repeated 20 U/kg dosing and no inhibitory anti‑C1‑INH antibodies.

Kalbitor (ecallantide) — EDEMA trials: double‑blind, placebo‑controlled studies (EDEMA‑3/EDEMA‑4 and pooled analyses) demonstrated significant symptom reduction at 4 hours vs placebo with maintained benefit through 24 hours; primary endpoints (MSCS, TOS) favored ecallantide and analyses showed low likelihood of clinically meaningful rebound/relapse.

Cinryze — Prophylaxis trials: a randomized, double‑blind, placebo‑controlled crossover trial (n=22) and an open‑label extension (n=146) support prophylactic dosing of 1000 U twice weekly, showing significantly reduced attack rates (e.g., 6.26 vs 12.73 attacks per 12‑week period in crossover trial) and large reductions in monthly attack rates in the extension (≈93.7% reduction).

Overall evidence context: randomized controlled and well‑conducted trials support efficacy of recombinant and plasma‑derived C1‑INH products and ecallantide for acute HAE treatment; Cinryze has randomized and extension data supporting prophylaxis but is not shown superior to other C1‑INH products for treatment in some plan types.

Background & definitions

Pathophysiology: Hereditary angioedema (HAE) due to C1‑INH deficiency/dysfunction leads to dysregulation of the contact (intrinsic coagulation) and kallikrein–kinin systems with excessive generation of bradykinin, a vasodilator that increases vascular permeability and produces the characteristic localized swelling and pain of HAE attacks.

Human C1‑INH (Berinert, Cinryze): plasma‑derived C1 esterase inhibitors are highly purified human proteins that replace deficient or dysfunctional C1 inhibitor, restoring regulation of complement, contact, and fibrinolytic pathways by inactivating plasma kallikrein and factor XIIa and thereby preventing bradykinin formation; Cinryze increases antigenic and functional plasma C1‑INH levels, and Berinert replaces the missing/malfunctioning protein for intravenous treatment.

Recombinant C1‑INH (Ruconest): rhC1INH is a serine protease inhibitor that irreversibly binds and inactivates proteases such as kallikrein and factor XIIa, suppressing complement and contact pathway activation and preventing bradykinin generation to reduce vascular permeability during attacks.

Plasma kallikrein inhibitor (Kalbitor/ecallantide): ecallantide is a potent, selective, reversible inhibitor of plasma kallikrein that binds kallikrein and blocks conversion of high‑molecular‑weight kininogen to bradykinin, directly reducing bradykinin generation and treating symptoms during acute HAE attacks.
