Initial Therapy — Medical Necessity Criteria

Desmopressin acetate nasal spray (Noctiva) is considered medically necessary when ALL of the following are met:

Primary eligibility: Patient is age 50 years or older.Age ≥ 50

Must meet all listed conditions for initial coverage.

Nocturnal polyuria confirmed: Documented diagnosis of nocturnal polyuria confirmed by 24-hour urine collection before treatment initiation.

Must meet ONE of the age-specific nocturnal urine volume thresholds; see next node.

Age-specific nocturnal urine volume thresholds

Nocturnal urine volume exceeds 20% of total 24-hour urine volume in an individual <65 years of age.
Confirm by 24-hour urine collection.
Nocturnal urine volume exceeds 33% of total 24-hour urine volume in an individual ≥65 years of age.
Confirm by 24-hour urine collection.

Symptom frequency: Individual awakens at least 2 times per night to void prior to initiating therapy.≥ 2 nightly voids

Baseline laboratory: Serum sodium concentration within the normal range (135 to 145 mmol/L).135–145 mmol/L

Contraindications absent: Prescriber has verified the individual does not have any of the following: currently receiving loop diuretics; currently receiving systemic or inhaled glucocorticoids; renal impairment with an eGFR <50 mL/min/1.73 m2; New York Heart Association class II–IV congestive heart failure; polydipsia; known or suspected syndrome of inappropriate antidiuretic hormone secretion.

Presence of any listed condition makes use not recommended.

Prior management: Patient has tried non-pharmacologic techniques or lifestyle interventions to manage nocturia (e.g., nighttime fluid restriction, avoidance of caffeine and alcohol, earlier timing of medications, leg elevation, compression stockings).

Prescriber specialty: Medication is prescribed by, or in consultation with, a urologist, geriatrician, or endocrinologist.

Continuation Therapy

Reauthorization / Continuation Therapy

Reauthorization: All initial medical necessity criteria remain met AND there is documentation of beneficial response to therapy.

Reauthorization approvals may be granted for up to 12 months.

Use of Noctiva for any indication other than nocturia due to nocturnal polyuria is considered experimental, investigational, or unproven. This includes use for primary nocturnal enuresis. Documentation and authorization for Noctiva will be denied when the request is for indications outside the approved adult indication of nocturia due to nocturnal polyuria.

Treatment of primary nocturnal enuresis with Noctiva is contraindicated and not medically necessary. Pediatric reports of hyponatremia-related seizures with intranasal desmopressin formulations and the absence of pediatric trial data support this contraindication.

Document attempted conservative, non‑pharmacologic interventions prior to initiating desmopressin (Noctiva).

Examples: nighttime fluid restriction, avoidance of caffeine and alcohol in evening, limiting total daily fluid intake, emptying bladder before bed, earlier timing of daytime diuretics/medications, leg elevation and/or use of compression stockings for peripheral edema, increased exercise or fitness measures

Denial Risk

Triggers for Denial / Contraindications

Therapy will be denied or is contraindicated in patients with conditions or situations that increase the risk of severe hyponatremia or adverse outcomes; use of Noctiva for primary nocturnal enuresis is contraindicated.

Uncontrolled hypertension or conditions with risk of fluid retention that would worsen underlying disease
Illnesses that may cause fluid or electrolyte imbalance (e.g., gastroenteritis, salt‑wasting nephropathies, systemic infection) — avoid initiation or hold therapy during such illnesses
Patients at risk for increased intracranial pressure or with history of urinary retention — generally not recommended or use with caution
