Medically Necessary (AlloMap)

Gene expression profile (i.e., AlloMap®) is considered medically necessary when ALL of the following criteria are met:

Patient is 15 years of age or older
Testing is performed between 2 months and 5 years post-heart transplantation
Test is performed in lieu of routinely scheduled endomyocardial biopsies
Results will be used to determine the need for subsequent endomyocardial biopsy to clarify rejection status
Operational: test should be used in conjunction with standard clinical evaluation (history/physical, echocardiography)
Stable heart allograft function must be demonstrated by ALL of the following:
- Absence of signs or symptoms of congestive heart failure (e.g., exertional dyspnea, orthopnea, PND, edema, syncope, palpitations/arrhythmias)
- Current assessment demonstrating left ventricular ejection fraction (LVEF) ≥ 45%
  LVEF may be assessed by echocardiography, cardiac catheterization, MRI, MUGA scan, or ventriculography
- Absence of severe cardiac allograft vasculopathy (CAV)
  CAV defined by angiography/IVUS criteria (e.g., ≥50% stenosis in a major epicardial vessel or maximal intimal thickness ≥0.5 mm on IVUS)

Background and Definitions

Organ transplant recipients face a risk of allograft rejection that has traditionally been detected by invasive biopsy. Noninvasive molecular tests such as gene expression profiling (e.g., AlloMap) and donor‑derived cell‑free DNA assays have been developed to aid surveillance and, in selected stable heart transplant recipients, to reduce routine endomyocardial biopsies. These tests are intended to be used in conjunction with standard clinical evaluation (history, physical exam, and graft function assessment) and have been studied only in defined, stable populations; their diagnostic performance and appropriate use depend on timing post‑transplant and patient selection.

Contraindications and Exclusions

AlloMap and other noninvasive surveillance strategies were evaluated in clinical trials that excluded patients with antibody‑mediated rejection or with hemodynamic compromise. The policy identifies these clinical states as contraindications to AlloMap use. In addition, study protocols excluded patients who had received a recent blood transfusion (30 days), recent high‑dose corticosteroids (20 mg), recent treatment for rejection, or other interventions that could alter circulating gene expression. When considering molecular testing, document timing relative to transplant and any recent transfusions or high‑dose steroid exposures as these factors were excluded from validation studies and may affect test validity.

Clinical trials and observational studies evaluating AlloMap excluded patients with clinical signs of declining graft function or active antibody‑mediated rejection, and often excluded patients with donor‑specific antibodies (DSA). Study protocols commonly required clinically stable recipients with acceptable LVEF thresholds and excluded recent high‑dose corticosteroid exposure (>20 mg), recent blood transfusion, and recent use of hematopoietic growth factors. These exclusions define the populations in which noninvasive surveillance was studied and inform appropriate candidate selection for test‑based monitoring.

VitaGraft Kidney has device‑specific contraindications and limitations. It is not indicated for patients who are pregnant, for subsequent testing performed within two weeks of transplant, for recipients of an allograft from an identical twin, for bone marrow or allogeneic stem cell transplant recipients, for recipients of non‑kidney/non‑liver organ transplants, or for recipients of multiple organ transplants. The test requires an initial urine baseline sample and is not FDA‑cleared; it is run in a CLIA‑certified, CAP‑accredited laboratory per the manufacturer.
