Infertility - Initial Therapy

Covered when ALL of the following are met

Infertility indication: Patient has infertility and requires inhibition of premature luteinizing hormone (LH) surges during controlled ovarian stimulation (COS) in conjunction with assisted reproductive procedures.

Approve for 1 year if met

Preferred product criteria must be met for the products listed; for Individual and Family Plans, Cetrotide (cetrorelix acetate 0.25 mg) brand is approved only if EITHER (A) the patient has tried generic cetrorelix 0.25 mg injection and cannot continue due to a formulation difference in inactive ingredient(s) that the prescriber states would cause a significant allergy or serious adverse reaction, OR (B) approval is to complete the current cycle if the patient has already started the current cycle on Cetrotide.

Injectable fertility medications, including GnRH antagonists, are specifically excluded under most benefit plans. Coverage for these injectable products depends on the member’s applicable benefit plan document; providers should verify plan-specific benefit availability and terms and conditions of coverage before initiating therapy.

The use of GnRH antagonists (cetrorelix acetate, Cetrotide, ganirelix acetate, Fyremadel) for any infertility indication other than inhibition of premature luteinizing hormone (LH) surge during controlled ovarian stimulation is considered experimental, investigational or unproven and therefore not covered.
