Initial Medical Necessity Criteria

Infertility indication: Patient has infertility and use is to inhibit premature luteinizing hormone (LH) surges in a woman undergoing controlled ovarian stimulation (COS) in conjunction with assisted reproductive procedures.

Supported by policy medical necessity criteria (see indication A).

Preferred product / generic-first: For Cetrotide (cetrorelix acetate 0.25 mg), the patient must have tried the bioequivalent generic cetrorelix acetate 0.25 mg and cannot take it due to a formulation difference in inactive ingredient(s) (e.g., dyes, fillers, preservatives) that, per the prescriber, would cause a significant allergy or serious adverse reaction.

Specifically, these agents are considered experimental, investigational, or unproven for ANY other infertility use outside their role in inhibiting premature luteinizing hormone (LH) surges during controlled ovarian stimulation.
