Coverage Criteria for Home Ventilators (E0466–E0468)

Non‑invasive interface ventilator (HCPCS E0466) — Initial therapy

Covered when ALL of the following are met

E0466 initial criteria

Eligible diagnoses: One of: neuromuscular disease; thoracic restrictive disease; chronic respiratory failure consequent to chronic obstructive pulmonary disease; congenital central hypoventilation syndrome; chronic lung disease of infancy (e.g., bronchopulmonary dysplasia); obesity hypoventilation syndrome; restrictive disorder of chest wall
Other requirements: Demonstrated failure of BPAP to improve hypercapnia and/or oxygen saturation level; does not require continuous (24 hours/day) ventilation

CONTINUED use: Pretreatment clinical condition met initial criteria AND documentation of compliant usage

Multifunction ventilator (HCPCS E0467)

Covered when ALL of the following are met

E0467 criteria: Meets the medical necessity coverage criteria for a home ventilator AND has a requirement for more than one of the four additional functions: oxygen concentrator, cough stimulator, suction pump, and nebulizer.

Dual‑function ventilator (HCPCS E0468) — Initial therapy

Covered when ALL of the following are met

E0468 criteria: Meets the medical necessity coverage criteria for a home ventilator AND has a requirement for cough stimulation.

Additional or duplicate devices

An additional home ventilator device is considered medically necessary for EITHER condition below

Medically necessary additional device: Condition 1 (Mobility): Individual meets the medical necessity coverage criteria for the specified home ventilator AND requires mechanical ventilation during mobility (e.g., for use in a wheelchair) as prescribed in the plan of care AND the primary ventilator is not portable or not suitable for wheelchair use; OR Condition 2 (In addition to negative pressure ventilator): Individual meets the medical necessity coverage criteria for the specified home ventilator AND a negative pressure ventilator is required for part of the day and a positive pressure ventilator is required during the rest of the day, as prescribed in the plan of care.

Medically necessary use of home ventilators

Considered medically necessary when criteria in the applicable policy statements are met:

Home ventilator coverage: Coverage for HCPCS codes E0466, E0467, and E0468 is provided when the specific medical necessity criteria detailed in the policy statements are satisfied.

Home ventilators provide positive pressure ventilatory support and may be used in the home to support or replace spontaneous breathing for individuals with respiratory failure or chronic ventilatory insufficiency. When used as a respiratory assistance device (RAD), a home ventilator supplies positive airway pressure via a non‑invasive interface (for example, a facemask) to improve ventilation and oxygenation; examples of RAD modalities include CPAP, APAP, BPAP/BiPAP, ASV, AVAPS, and iVAPS. Coverage under this policy is limited to situations where the device is functioning as a home ventilator to treat life‑threatening or chronic respiratory insufficiency according to the specific medical necessity criteria for HCPCS codes E0466, E0467, and E0468 rather than solely as a PAP‑type RAD.

Note: The policy statement for an invasive interface has been removed and is no longer supported by this policy (Annual Review entry dated 6/15/2025).

A home ventilator (HCPCS E0466–E0468) is considered not medically necessary when used for non‑life‑threatening conditions or when its sole purpose is to function as a RAD—including continuous positive airway pressure (CPAP), auto‑titrating PAP (APAP), bilevel positive airway pressure (BPAP/BiPAP), adaptive servo‑ventilation (ASV), average volume assured pressure support (AVAPS), or intelligent volume assured pressure support (iVAPS). Use of a home ventilator exclusively for treatment of obstructive sleep apnea is also not medically necessary under this policy.

Coding and billing note: Services submitted with deleted or inactive procedure codes or codes that are not effective at the time the service is rendered may be ineligible for reimbursement. Ensure that HCPCS and other billing codes are current and active at the date of service to avoid claim denials.
