Cigna pharmacy prior authorization policy for Nucala (mepolizumab subcutaneous injection) detailing coverage criteria for FDA‑approved indications (asthma, COPD, CRSwNP, EGPA, HES), not‑covered uses, prescriber requirements, approval durations, and prior therapy/eosinophil thresholds.
