Intrapulmonary percussive ventilation (IPV) delivers high‑frequency mini‑bursts of gas and therapeutic aerosols to mobilize secretions and is primarily designed for inpatient/hospital use. The literature includes small, heterogeneous studies, case series, and limited randomized trials, and systematic reviews conclude results are insufficient and inconsistent to support routine outpatient or home use.

As a result of the limited and heterogeneous evidence base, routine authorization of IPV for home use is considered not medically necessary and requests for home IPV are at higher risk for denial unless stronger, specific supportive evidence is provided.

Several device‑specific policy statements were removed in the recent revisions. The policy no longer contains separate statements for acoustical percussors, combination PEP and aerosol drug delivery systems, mechanical percussors, or oscillatory (vibratory) PEP devices; these removals are documented in the revision notes.

Mechanical insufflation‑exsufflation (MI‑E; HCPCS E0482) is an established airway clearance modality that alternates positive and negative airway pressures to generate high expiratory flow and augment cough. This policy limits MI‑E coverage to patients with neuromuscular disorders (for example, muscular dystrophy, multiple sclerosis, amyotrophic lateral sclerosis) who have significant impairment of chest wall and/or diaphragmatic movement resulting in difficulty clearing secretions.

Use of MI‑E for any indication other than the neuromuscular conditions listed in the policy is not covered.

IPV devices are FDA‑cleared Class II devices intended to mobilize secretions primarily in inpatient settings, but the evidence supporting safety and efficacy for routine home airway clearance is limited, heterogeneous, and derives largely from small studies and case series.

Accordingly, IPV for routine home use is considered not medically necessary under this policy (HCPCS E0481) and outpatient/home authorization should not be routinely granted given the current evidence base.
