Prescriber-documented clinical concern: According to the prescriber, there is a significant clinical concern such that the patient is unable to use generic prenatal vitamins.n/a

Any use of prescription prenatal vitamin supplements outside the indications and criteria stated in this policy is considered experimental, investigational, or unproven. Criteria in this policy will be updated as new published data become available.

Receipt of sample product does not satisfy criteria.
