Product-specific coverage criteria

Covered when ALL of the following are met (per product-specific rules and quantity limits):

Thalomid 50 mg coverage

If dispensed at retail, requested quantity does not exceed 196 capsules per dispensing (override for ENL).
If dispensed via home delivery, requested quantity does not exceed 588 capsules per dispensing (override for ENL).
Requested regimen is consistent with the approval note allowing a dose equivalent to 350 mg once daily for 28 days at retail or 350 mg once daily for 84 days at home delivery.

Thalomid 100 mg coverage: Request is for Thalomid 100 mg capsules and does not exceed the standard quantity limits per dispensing: retail maximum 112 capsules and home delivery maximum 336 capsules.retail<=112, home<=336

No clinical overrides are provided for 100 mg strength.

Any exception that does not meet the Drug Quantity Management rule and is not explicitly covered by the product-specific criteria is considered not medically necessary. All approvals under this Drug Quantity Management program are provided for 1 year when Drug Quantity Management criteria are met. If the Drug Quantity Management rule is not met at the point of service, coverage will be determined by the policy criteria and may result in denial or request for additional documentation. Overrides to standard quantity limits are allowed only as specified in the Drug Quantity Limits and Criteria sections (for example, allowed dispensing quantities per Rx for Thalomid 50 mg and 100 mg as detailed in the policy).

Approvals duration: 1 year when criteria met.
If quantity rule not met at point of service, follow policy criteria for coverage determination; expect denials or requests for more information.
