Bynfezia Pen exception: Approve for 1 year if BOTH: (A) patient meets the standard Somatostatin Analogs - Octreotide Immediate-Release Products Prior Authorization Policy criteria; AND (B) patient has tried generic octreotide acetate immediate-release injection.1 year

If the patient meets A but not B, alternate handling described in policy (see Sandostatin handling).

Sandostatin exception: Approve for 1 year if BOTH: (A) patient meets the standard Somatostatin Analogs - Octreotide Immediate-Release Products Prior Authorization Policy criteria; AND (B) patient has tried generic octreotide acetate immediate-release injection.1 year

If the patient meets A but not B, approve generic octreotide acetate immediate-release injection.

Any exception request for a non-preferred octreotide immediate‑release product will be reviewed against the specific non‑preferred product exception criteria in this policy. Requests that meet the stated exception criteria will be approved per the criteria (see product-specific rules). Any exception requests for non‑preferred products that do not meet the stated non‑preferred product exception criteria are considered not medically necessary.

Non‑preferred octreotide products (for example, Bynfezia Pen and Sandostatin) are considered not medically necessary unless the applicable non‑preferred product exception criteria are met. When the exception criteria are satisfied, coverage may be approved in accordance with the product‑specific rules (see exception criteria).
