EULAR and ACR considerations

Topical NSAIDs — hip OA: Topical NSAIDs are not expected to be efficacious in hip OA due to the depth of the joint; oral NSAIDs are preferred.

Oral NSAIDs — general: Oral NSAIDs are strongly recommended as mainstay pharmacologic therapy for OA but should be used at the lowest effective dose for the shortest duration due to GI, cardiovascular, renal, and heart failure risks.

Widespread joint involvement: When many joints are affected, monitor cumulative topical dosing to avoid exceeding recommended doses; oral therapy may be preferred for widespread disease.

As of the policy revision effective 7/15/2025, the following combination products have been removed from the policy and are excluded from coverage under this document: Vimovo (naproxen/esomeprazole), Duexis, and ibuprofen/famotidine. These products will not be approved for coverage under this policy following the revision date.

Background

All NSAIDs carry cardiovascular and gastrointestinal safety warnings that should inform prescribing and coverage decisions.

Clinical guideline recommendations differ by joint and clinical context: topical NSAIDs are strongly recommended for knee osteoarthritis and are preferred for hand OA in many guideline statements due to a more favorable local safety profile, whereas oral NSAIDs remain the mainstay pharmacologic therapy for hip OA and for broader or multi-joint disease. Guidelines cited in this policy include ACR (2019), EULAR (2018), and OARSI (2019).
