Atopic Dermatitis

A) Initial Therapy: Approve for 4 months if patient is ≥ 6 months of age AND has atopic dermatitis involvement estimated to be ≥ 10% body surface area AND has tried at least one medium- to super-high‑potency prescription topical corticosteroid applied daily for ≥ 28 consecutive days with inadequate efficacy; AND medication is prescribed by or in consultation with an allergist, immunologist, or dermatologist.

Initial authorization duration for atopic dermatitis is 4 months (updated).

B) Currently Receiving Dupixent: Approve for 1 year if patient has already received ≥ 4 months of Dupixent AND has responded to therapy as determined by the prescriber (examples: marked improvements in erythema, induration/papulation/edema, excoriations, lichenification; reduced pruritus; decreased requirement for other therapies; reduced body surface area affected).

Bullous Pemphigoid

A larger study of Dupixent combined with Palforzia showed modest increases in DBPCFC responders but did not reduce Palforzia‑related anaphylaxis; the policy therefore concludes evidence is insufficient to support routine coverage for peanut allergy.

The policy considers concurrent use of Dupixent with another monoclonal antibody therapy or with Janus kinase (JAK) inhibitors (oral or topical) to be not medically necessary.
