Coverage Criteria

inv-01: Marinol (initial therapy)

Marinol is covered when the following FDA‑approved indication criteria are met:

Marinol (dronabinol capsules) FDA‑approved indications: 1) Anorexia associated with weight loss in a patient with AIDS: approve for 6 months if the patient meets Preferred product criteria listed below. 2) Nausea and vomiting associated with cancer chemotherapy: approve for 1 year if BOTH A and B are met; A) patient has failed to respond adequately to at least TWO conventional antiemetic treatments; AND B) Preferred product criteria is met.

Preferred product criteria (Employer and Individual/Family plans): patient has tried bioequivalent generic dronabinol capsules and cannot take the generic due to a formulation difference in inactive ingredient(s) (e.g., dyes, fillers, preservatives) that, per the prescriber, would result in a significant allergy or serious adverse reaction.

inv-02: Syndros (initial therapy)

Syndros is covered when the following FDA‑approved indication criteria are met:

Syndros (dronabinol oral solution) FDA‑approved indications: 1) Anorexia associated with weight loss in a patient with AIDS: approve for 6 months if the patient meets Preferred product criteria listed below. 2) Nausea and vomiting associated with cancer chemotherapy: approve for 1 year if BOTH A and B are met; A) patient has failed to respond adequately to at least TWO conventional antiemetic treatments; AND B) Preferred product criteria is met.

Preferred product criteria for Syndros (Employer and Individual/Family plans): patient has tried generic dronabinol capsules OR patient cannot swallow or has difficulty swallowing capsules (plan-specific for some plan types).
