Hypercalcemia due to Parathyroid Carcinoma: A) Medication prescribed by or in consultation with an oncologist or endocrinologist; AND B) Preferred product criteria is met for the product as listed in the preferred product table.

Preferred Product / Step Therapy Requirement for IFP

For Individual and Family Plans, a trial of the bioequivalent generic cinacalcet tablet is required before coverage of the brand product Sensipar, unless the prescriber documents that the patient cannot take the generic because of a formulation-related intolerance or allergy (e.g., inactive ingredient difference causing a significant allergic reaction or serious adverse event). Documentation of the intolerance/allergy should be included with the authorization request.
