Step Therapy Criteria
Covered when ALL of the following step therapy criteria are met
Primary step therapy logic
- Step 1 trial requirement: Patient has tried one Step 1 product (see Step 1 list)
If met, approve Step 2 product
- Swallowing difficulty exception: Patient cannot swallow or has difficulty swallowing tablets
If met, approve valsartan oral solution
If the Step Therapy rule is not met for a Step 2 product at point of service, coverage will be determined by these criteria
