Covered when ALL of the following are met:
Presbyopia is characterized as a normal age-related change rather than an illness, injury, disease, or its symptoms; therefore treatment is not medically necessary.
Treatment of presbyopia with Vuity is explicitly listed as a condition not covered. Any use of Vuity for the indication of presbyopia is considered not medically necessary under this policy.
The policy rationale states that although Vuity is FDA-approved for presbyopia in adults, coverage is limited to treatments required to diagnose or treat an illness, injury, disease or its symptoms.
