Transvaginal ultrasound (TVUS, also TVU or TVS) involves insertion of a transducer into the vagina to obtain high‑resolution pelvic images of the uterus, ovaries, and fallopian tubes and generally provides greater anatomic detail than transabdominal ultrasound.

The U.S. Food and Drug Administration regulates ultrasound systems and transducers as Class II devices via the 510(k) process; these devices are indicated for evaluation of pelvic organs and are intended for use by qualified personnel.

Professional organizations cite TVUS as appropriate for diagnostic evaluation of suspected pelvic pathology — for example, initial assessment of postmenopausal bleeding, characterization and localization of pelvic masses, evaluation of acute pelvic pain, assessment of uterine fibroids, and monitoring during controlled ovarian hyperstimulation — but emphasize that TVUS is not supported as an effective population screening tool for ovarian or endometrial cancer due to poor performance for cancer detection and risk of false positives leading to unnecessary interventions.

Routine screening for ovarian cancer using TVUS and/or serum CA-125 is not recommended for asymptomatic women at average risk due to lack of mortality benefit and risk of false positives leading to unnecessary invasive procedures.
For individuals with hereditary risk (e.g., BRCA1/2), there is no recommendation for routine TVUS screening for ovarian cancer; CA-125 and pelvic ultrasound may be used for preoperative planning in BRCA-positive individuals undergoing risk-reducing surgery.
For individuals with hereditary syndromes associated with uterine (endometrial) cancer (e.g., Lynch syndrome and listed variants), routine TVUS for endometrial cancer screening lacks sufficient sensitivity and specificity and is not routinely recommended; it may be considered at clinician discretion, particularly in postmenopausal patients.
