Initial approval — Parkinson's Disease

Covered when ALL of the following are met:

FDA-Approved Indication for Parkinson's Disease: Approve for 1 year if the patient meets ALL of the following (A, B, C, and D)

A: Off episodes: Patient is experiencing 'off' episodes (examples include muscle stiffness, slow movements, or difficulty starting movements)

B: Concomitant therapy: Patient is currently receiving carbidopa/levodopa therapy

C: Prior trial or intolerance of other MAO-B inhibitors

Documentation must include: diagnosis of Parkinson's disease with evidence of 'off' episodes; current use of carbidopa/levodopa; prior trial of oral selegiline (tablets or capsules) or rasagiline with outcome (intolerance) or documentation of swallowing difficulty if skipping prior trial; and that the prescriber is a neurologist or that the neurologist was consulted.
