Defines Cigna's prior authorization criteria and coverage stance for Qulipta (atogepant) for migraine prevention in adults under Cigna-administered health benefit plans.
Qulipta is indicated for both episodic and chronic migraine prevention; approval criterion changed to require ≥ 4 migraine headache days per month (removed upper bound of <15).
Removed requirement that patients try at least two standard prophylactic pharmacologic therapies from different classes prior to Qulipta.
Concurrent use of Qulipta with another CGRP inhibitor for migraine prevention is considered experimental and not covered.
Coverage Criteria for Qulipta (atogepant)
Migraine Headache Prevention
Covered when ALL of the following are met:
Approve for 1 year when all conditions are met. Examples of significant clinical benefit include reduction in overall migraine days per month or reduction in number of severe migraine days per month from the time Qulipta was initiated.
Requests for the concurrent use of Qulipta (atogepant) with another calcitonin gene-related peptide (CGRP) inhibitor prescribed for migraine headache prevention are considered experimental, investigational, or unproven and are not covered. The policy specifically identifies CGRP agents used for migraine prevention (Aimovig, Ajovy, Emgality, Vyepti, Nurtec ODT, and Qulipta) and notes that injectable agents have not been studied in combination with another CGRP-targeting therapy.
Use of Qulipta for any indication other than the FDA-approved migraine prevention indication is considered experimental, investigational, or unproven and is not covered by this policy.
