Coverage Criteria for Miplyffa (arimoclomol)

Initial Therapy

Covered when ALL of the following are met for FDA-approved indication (Niemann-Pick disease type C). Approvals provided for 1 year.

Initial Therapy (Approve for 1 year): Patient is ≥ 2 years of age; AND patient has one or more neurological symptom(s) of Niemann-Pick disease type C (examples include loss of motor function, swallowing, speech, or cognitive impairment); AND patient can walk independently or with assistance; AND diagnosis is established by genetic testing showing biallelic pathogenic variants in either NPC1 or NPC2; AND patient does NOT have adult-onset Niemann-Pick disease type C (defined as age of first neurologic symptom > 15 years); AND the patient meets ONE of: (a) medication will be taken in combination with miglustat OR (b) prescriber documents that patient is unable to take miglustat; AND medication is prescribed by or in consultation with a geneticist, endocrinologist, metabolic disorder subspecialist, neurologist, or physician who specializes in NPC.

Approve for 1 year.

Continuation Therapy (Currently Receiving)

Covered for patients currently receiving Miplyffa when ALL of the following criteria are met:

Currently Receiving: Patient does NOT have adult-onset Niemann-Pick disease type C (defined as age of first neurologic symptom > 15 years); AND the patient meets ONE of: (a) medication will be taken in combination with miglustat OR (b) prescriber documents that patient is unable to take miglustat; AND prescriber documents that the patient has derived benefit from treatment defined as disease stabilization, slowed progression, or improvement; AND medication is prescribed by or in consultation with a geneticist, endocrinologist, metabolic disorder subspecialist, neurologist, or physician who specializes in NPC.

Continuation approval contingent on documented benefit per prescriber.

Miplyffa (arimoclomol) is not covered for uses outside the authorization criteria and is considered experimental, investigational, or unproven for any other indication. Examples of noncovered or investigational uses include: amyotrophic lateral sclerosis (ALS), combination use with Aqneursa (levacetylleucine), Gaucher disease, and inclusion body myositis. The policy also states that coverage is not recommended for circumstances not explicitly listed in the Recommended Authorization Criteria, and the criteria will be updated as new published data become available.
