Initial therapy (FDA-approved indication)

Pulmonary conditions contraindicate use

D: Inbrija is prescribed by or in consultation with a neurologist or physician who specializes in the condition

Specialist involvement required

Inbrija for any use other than the FDA-approved indication — intermittent treatment of "off" episodes in Parkinson's disease for patients who are taking carbidopa-levodopa — is considered not medically necessary. The policy requires that the patient meet all specified coverage criteria (including specialist involvement and pulmonary health) for approval; uses outside that indication are excluded from coverage.

When coverage is available and medically necessary, the dosage, frequency, duration of therapy, and site of care should be reasonable, clinically appropriate, and supported by evidence-based literature. Receipt of sample product does not satisfy any criteria requirements for coverage.

Rapid-onset levodopa delivery (the drug class that includes inhaled levodopa formulations) has limited evidence for treatment of motor fluctuations and is noted in cited guideline reviews to have insufficient evidence and may be considered investigational for that indication.

Prior Authorization is required for prescription benefit coverage of Inbrija. All approvals are provided for the duration noted by the plan. Because of the specialized skills required for evaluation and diagnosis of patients treated with Inbrija and the monitoring required for adverse events and long-term efficacy, approval requires Inbrija to be prescribed by or in consultation with a physician who specializes in the condition being treated.

Approval period: Approve for 1 year when criteria are met.

When billing, providers must use the most appropriate codes as of the effective date of the submission. Claims submitted for services that are not accompanied by covered code(s) under the applicable Coverage Policy will be denied as not covered.

Use current, most-appropriate CPT/HCPCS/ICD-10 codes effective on date of submission.
