Coverage Criteria for Carbidopa (Lodosyn and Generic)

FDA-Approved Indications — Initial and Renewal Authorization

Approve for 1 year if ALL of the following are met for each FDA-approved indication:

Indication requirement: Patient has one of the FDA‑approved indications: Parkinson's disease (idiopathic), postencephalitic parkinsonism, or symptomatic parkinsonism.

Current therapy: Patient is currently receiving carbidopa/levodopa therapy.

Prescriber specialty: Medication is prescribed by or in consultation with a neurologist (physician who specializes in the condition).

Specialist involvement required due to evaluation, diagnosis, and monitoring needs.

Preferred product criteria for brand Lodosyn: For brand product coverage, the patient has tried the bioequivalent generic carbidopa tablet AND cannot take it because a formulation difference in inactive ingredient(s) (e.g., dyes, fillers, preservatives) would, per the prescriber, cause a significant allergy or serious adverse reaction.

Receipt of a sample product does not satisfy any coverage criteria or prior authorization requirements. Providers must document an eligible prescription and supporting clinical information per the policy to qualify for coverage.

Use of carbidopa is limited to the FDA-approved indications listed in this policy: Parkinson's disease (idiopathic), postencephalitic parkinsonism, and symptomatic parkinsonism. Carbidopa prescribed for any indication other than these FDA-approved uses is considered not medically necessary and is excluded from coverage.

Background

Carbidopa is an aromatic amino acid decarboxylase inhibitor that is indicated for use in combination with levodopa to treat Parkinsonian syndromes, including idiopathic Parkinson's disease, postencephalitic parkinsonism, and symptomatic parkinsonism. In clinical use it prevents peripheral conversion of levodopa to dopamine, allowing more levodopa to reach the central nervous system when given together.
