Covered when criteria below are met for hyponatremia:

Hyponatremia approval pathways: Approve when ONE of the following pathways (Initial Therapy OR Currently Receiving Tolvaptan) is met

Initial Therapy: Approve for 30 days if ALL of the following are met

Initial therapy pathway applies to patients starting tolvaptan or who have received <30 days

Initial Therapy - elements: (1) Patient is >= 18 years of age; AND (2) Patient meets ONE of: (a) baseline serum sodium <125 mEq/L; OR (b) baseline serum sodium <135 mEq/L AND symptomatic hyponatremia (e.g., nausea, vomiting, headache, lethargy, confusion); OR (c) patient has already been started on tolvaptan and has received <30 days of therapy (approve to complete 30 total days)

Specifically, use of Samsca for Autosomal Dominant Polycystic Kidney Disease (ADPKD) is not covered; Jynarque® (a different tolvaptan product) is the indicated agent for slowing kidney function decline in adults with rapidly‑progressing ADPKD and has different recommended dosing and REMS considerations.

Concomitant use of Samsca with Jynarque is not recommended and is considered not medically necessary.

Tolvaptan (Samsca or generic) is not medically necessary for patients who require urgent intervention to raise serum sodium to prevent or treat serious neurologic symptoms, because Samsca has not been studied for acute correction of serum sodium in urgent settings.

Patients requiring urgent/rapid correction of serum sodium to prevent or treat serious neurological symptoms — tolvaptan has not been studied for acute urgent correction and is not covered in that setting
Any other use not supported by the policy or published evidence will be considered not medically necessary

Background

Tolvaptan is a selective vasopressin V2‑receptor antagonist indicated for the treatment of clinically significant hypervolemic and euvolemic hyponatremia. It acts by antagonizing the V2 receptor to promote aquaresis and increase serum sodium.

Clinical trial data (SALT‑1 and SALT‑2) demonstrated tolvaptan increases serum sodium in patients with hyponatremia, but symptomatic benefit for all patients is not established and tolvaptan is not appropriate for urgent correction of severe neurologic hyponatremia.

Prior authorization is recommended for tolvaptan for hyponatremia and approvals are provided for specified durations depending on whether the request is for initial therapy or for continuation in patients already established on therapy.
