Tiopronin products (Thiola, Thiola EC, Venxxiva) are covered with prior authorization for the prevention of cystine kidney stones in adults and pediatric patients ≥ 20 kg who have severe homozygous cystinuria and are unresponsive to conservative measures. Approvals are granted for 1 year. Therapy must be prescribed by or in consultation with a nephrologist, urologist, or a physician who specializes in treatment of cystinuria.

Background: Tiopronin is indicated, in combination with high fluid intake, urinary alkalization, and diet modification, for prevention of cystine kidney stone formation in adults and pediatric patients ≥ 20 kg with severe homozygous cystinuria unresponsive to these measures. Tiopronin increases urinary cystine solubility by binding cystine. Recommended supportive measures include large-volume fluid intake, urine alkalization (e.g., potassium citrate), and dietary sodium and animal protein restriction; guidelines recommend conservative measures be tried first.

Definitions: Cystinuria: Autosomal recessive disorder of abnormal cystine transport leading to excessive urinary cystine and recurrent kidney stones.
