Oral cysteamine products are considered medically necessary when ONE of the following is met (I or II):

I. Procysbi (FDA-Approved Indication)

A: Patient is > 1 year of age

B: Diagnosis confirmed by ONE of: (i) genetic testing confirmed biallelic pathogenic or likely pathogenic variants in the CTNS gene [documentation required] OR (ii) white blood cell cystine concentration above the upper limit of the normal reference range for the reporting laboratory [documentation required]

C: Patient will not be using Cystagon and Procysbi concurrently

D: Medication prescribed by or in consultation with a nephrologist or a metabolic disease specialist (or specialist who focuses in the treatment of metabolic diseases)

Oral cysteamine products are not medically necessary for any indication other than nephropathic cystinosis. This exclusion applies to all oral cysteamine formulations and indications not explicitly listed as covered in the policy. Documentation submitted for coverage of oral cysteamine products for non‑nephropathic uses will be denied as not covered.

Step Therapy Requirement for Procysbi (Individual & Family Plans)

Procysbi (cysteamine bitartrate delayed-release) for Individual & Family Plans requires that the patient has tried Cystagon (cysteamine bitartrate capsules) and, according to the prescriber, had inadequate efficacy or significant intolerance to Cystagon. Documentation of this trial and outcome is required.

Cystinosis is a rare, inherited metabolic disorder in which cystine accumulates within lysosomes, leading to multi‑organ damage. Diagnosis of the nephropathic form is made using laboratory and genetic confirmation methods and guides use of cystine‑depleting therapy.
