Coverage Criteria for HIV Antiretroviral (ARV) Products

Medical Necessity / Prior Authorization

HIV Products are considered medically necessary for the treatment of HIV infection when ANY of the following criteria are met; prior authorization is required for products listed in Appendix 1.

Medical necessity (general): HIV Products are considered medically necessary for the treatment of HIV infection when ANY of the following are met. Prior authorization criteria for products listed in Appendix 1 apply as specified by the member's employer group plan.

See Appendix 1 and the customer's benefit plan document for product- and plan-specific prior authorization requirements.

Prior authorization requirement: Requests for HIV products that are included on Appendix 1 require prior authorization per the plan-specific Cigna drug lookup (Appendix 1).

Appendix 1 lists product-specific PA requirements and may vary by customer; verify coverage and criteria on the Cigna for Health Care Professionals site.

Preferred-regimen sequencing for certain multi-ingredient regimens: For certain multi-ingredient regimens, coverage may require the individual is not a candidate for five preferred single-tablet complete regimens (examples: Biktarvy, Dovato, generic Symfi/Symfi Lo, Genvoya, Symtuza, Triumeq) before approval. Refer to the customer's benefit plan document for specifics.

The list of preferred single-tablet regimens and applicability is plan-dependent.

Non-Covered Product Approval Conditions

Certain historically listed or non-covered HIV products may be approved only when specified criteria are met (examples and common approval conditions shown):

Examples of approval conditions for non‑covered or historically listed products: Approval for non-covered or historically listed HIV products is granted only when the specific condition listed for the product is met (examples include documented intolerance to a generic formulation; documented trial of the bioequivalent generic product with inability to use it due to formulation differences in inactive ingredients causing significant allergy or serious adverse reaction; established prior treatment on the brand product; or the individual is not a candidate for other antiretroviral agents).

Each product table entry in the policy lists the precise condition (intolerance, trial of generic, established treatment, or not a candidate for other agents). Documentation supporting the condition is required.

Documented intolerance to generic formulation: The member has documented intolerance to the specified generic formulation of the drug (e.g., documented intolerance to a generic formulation of Combivir, Norvir, Retrovir, Viread, Ziagen, etc.).
