Saphnelo efficacy has not been evaluated and is not recommended for patients with severe active lupus nephritis or severe active central nervous system (CNS) lupus. This limitation reflects the absence of clinical evaluation in these manifestations of systemic lupus erythematosus and should guide therapy selection and prior authorization determinations.

Concurrent use of Saphnelo with other biologic therapies is not recommended; safety and efficacy of such combinations have not been established. Examples include, but are not limited to, intravenous or subcutaneous belimumab (Benlysta) and rituximab. Refer to the policy Appendix for additional biologics that should not be administered in combination with Saphnelo.

Any use of Saphnelo outside the indications and criteria described in this policy is considered experimental, investigational, or unproven. This includes combination therapy with other biologic agents for which safety and efficacy in combination with Saphnelo have not been demonstrated.

Providers should not request coverage for unlisted or novel indications, off‑label combinations with biologics, or other uses not supported by the policy; such requests are expected to be denied as not covered until supporting evidence is available and the policy is updated.

Prior Authorization is recommended for prescription benefit coverage of Saphnelo. Initial approvals for new therapy are for 6 months; continuation approvals for patients currently receiving Saphnelo are for 1 year. Because of the specialized skills required for evaluation, diagnosis, and monitoring of patients treated with Saphnelo, the medication must be prescribed by a physician who has consulted with or who specializes in the condition (rheumatologist, clinical immunologist, nephrologist, neurologist, or dermatologist).

Background on Saphnelo (anifrolumab-fnia)

Saphnelo (anifrolumab‑fnia) is a type I interferon receptor antagonist indicated for the treatment of moderate to severe systemic lupus erythematosus (SLE) in adults as an adjunct to standard therapy. The agent targets the type I interferon pathway, which is implicated in SLE pathogenesis, and is administered as 300 mg IV every 4 weeks per the approved dosing schedule.

Prior authorization is recommended for Saphnelo. Initial approvals are provided for 6 months when criteria for initiation are met; continuation approvals for patients already receiving Saphnelo are provided for 1 year. Saphnelo has not been evaluated in severe active lupus nephritis or severe active CNS lupus and is not recommended in those settings.
