Concurrent use of Lupkynis with biologic agents or with cyclophosphamide is considered experimental/unproven and is not covered. The policy notes that Lupkynis has not been studied in combination with other biologics or with cyclophosphamide, and that safety and efficacy have not been established for these combinations.

Because these combinations are unstudied, claims that propose concurrent use with biologics or cyclophosphamide may be denied as not covered; providers should avoid these combinations unless supported by new, peer-reviewed evidence and appropriate authorization.

Use of Lupkynis (voclosporin) for treatment of plaque psoriasis is not covered. In a Phase III trial voclosporin was found to be inferior to cyclosporine, an established therapy for plaque psoriasis, and multiple other FDA‑approved therapies with established efficacy are available.

Any use of Lupkynis beyond the FDA‑approved indication for active lupus nephritis in adults is considered experimental, investigational, or unproven and is not covered. The policy’s list of "Conditions Not Covered" includes examples but is not exhaustive and will be updated as new published data become available.

Prior Authorization

Prior Authorization and Approval Durations

Prior Authorization is required for Lupkynis (voclosporin). Initial approvals are provided for 6 months; continuation (patients currently receiving Lupkynis) approvals are provided for 1 year. All authorization durations are calendar-month based (1 month = 30 days).

Required Prescriber and Clinical Documentation

Lupkynis must be prescribed by or in consultation with a nephrologist or rheumatologist. Clinical documentation supporting the request should include: diagnosis confirmation (renal biopsy confirming lupus nephritis class, when applicable), patient age, baseline and current eGFR, description of concurrent background immunosuppressive regimen (for example, mycophenolate mofetil or azathioprine with a systemic corticosteroid), and for continuation requests, documentation of clinical response as determined by the prescriber (examples: improvement in organ dysfunction, reduction in flares, reduction in corticosteroid dose, decrease in anti-dsDNA titer, improvement in complement levels).

Background

Lupkynis (voclosporin) is a calcineurin inhibitor immunosuppressant indicated in combination with background immunosuppressive therapy for the treatment of active lupus nephritis in adults. The agent is intended to be used alongside a background immunosuppressive regimen (for example, mycophenolate mofetil or azathioprine plus systemic corticosteroid) and is administered under the supervision of a nephrologist or rheumatologist.

KDIGO guidelines list Lupkynis as an initial treatment option for biopsy‑confirmed active Class III or IV (± V) lupus nephritis; the policy requires pathologic confirmation (e.g., WHO class III/IV/V) for initial therapy and specifies a renal function threshold of eGFR > 45 mL/min/1.73 m2 for initial approval, with prior authorization recommended and approval durations defined for initial and continuing therapy.
