Coverage Criteria for Cibinqo (abrocitinib)

Initial and Continuation Therapy

Covered when ONE of the following paths is met:

Atopic Dermatitis approval paths: Approve when either Initial Therapy OR Current Therapy criteria are met

Initial Therapy: Patient is >= 12 years of age; AND patient meets ONE of the following: (a) patient has had a 4-month trial of at least ONE systemic therapy; OR (b) patient has tried at least ONE systemic therapy but was unable to tolerate a 4-month trial; AND medication is prescribed by or in consultation with an allergist, immunologist, or dermatologist.Initial approval duration = 3 months

Examples of systemic therapies include dupilumab, lebrikizumab, nemolizumab, tralokinumab, methotrexate, azathioprine, cyclosporine, and mycophenolate mofetil.

Current (Continuation) Therapy: Patient has already received at least 90 days of therapy with Cibinqo; AND patient experienced a beneficial clinical response compared with baseline in at least one sign measure (e.g., estimated body surface area affected, erythema, induration/papulation/edema, excoriations, lichenification, or decreased requirement for other topical or systemic therapies); AND patient experienced improvement in at least one symptom (e.g., decreased itching).Continuation approval duration = 1 year

A patient who has received < 90 days of therapy or who is restarting therapy should be considered under Initial Therapy.

Requests for concurrent administration of Cibinqo with a biologic used for an inflammatory condition, another JAK inhibitor, or other potent immunosuppressants are not recommended due to safety concerns and lack of controlled trial data demonstrating additive efficacy. Such combination use may increase the rate of adverse effects and is listed as a condition not covered under this policy.
