Lupus - Benlysta (belimumab) subcutaneous prior authorization
Defines Cigna's prior authorization requirements and medical necessity criteria for coverage of Benlysta subcutaneous in patients with systemic lupus erythematosus (SLE) or lupus nephritis, and specifies prescriber specialty and concomitant therapy expectations.
For initial therapy of lupus nephritis, a requirement was added that the patient has biopsy-confirmed lupus nephritis.
Requirement for concurrent therapy was broadened from 'standard therapy' to 'an immunosuppressive regimen' and examples were expanded to include leflunomide, methotrexate, and/or systemic corticosteroids.
The exception for patients intolerant to standard therapy due to significant toxicity as determined by the prescriber was removed from the policy.
