POLICY UPDATE CHANGES

For initial therapy of lupus nephritis, a requirement was added that the patient has biopsy-confirmed lupus nephritis.

Requirement for concurrent therapy was broadened from 'standard therapy' to 'an immunosuppressive regimen' and examples were expanded to include leflunomide, methotrexate, and/or systemic corticosteroids.

The exception for patients intolerant to standard therapy due to significant toxicity as determined by the prescriber was removed from the policy.

Coverage Criteria — Benlysta (belimumab) Subcutaneous

inv-01: Lupus Nephritis — Covered for lupus nephritis when ONE of the following is met

Covered for lupus nephritis when ONE of the following is met

Lupus Nephritis: Initial or Current Therapy: A) Initial Therapy: Approve for 6 months if ALL of (i) patient is ≥ 5 years of age; (ii) diagnosis of lupus nephritis has been confirmed on biopsy (for example, WHO class III, IV, or V); (iii) the medication is being used concurrently with an immunosuppressive regimen (examples include azathioprine, cyclophosphamide, leflunomide, methotrexate, mycophenolate mofetil and/or a systemic corticosteroid); and (iv) the medication is prescribed by or in consultation with a nephrologist or rheumatologist. OR B) Patient is Currently Receiving Benlysta Subcutaneous or Intravenous: Approve for 1 year if ALL of (i) the medication is being used concurrently with an immunosuppressive regimen (examples include azathioprine, cyclophosphamide, leflunomide, methotrexate, mycophenolate mofetil and/or a systemic corticosteroid); (ii) the medication is prescribed by or in consultation with a nephrologist or rheumatologist; and (iii) the patient has responded to Benlysta subcutaneous or intravenous, as determined by the prescriber (examples of response include improvement in organ dysfunction, reduction in flares, reduction in corticosteroid dose, decrease of anti-dsDNA titer, and improvement in complement levels).

inv-02: Systemic Lupus Erythematosus (SLE) — Covered for SLE when ONE of the following is met

SLE: Initial or Current Therapy: A) Initial Therapy: Approve for 4 months if ALL of (i) patient is ≥ 5 years of age; (ii) patient has autoantibody-positive SLE (positive ANA and/or anti-dsDNA); (iii) patient meets ONE of (a) the medication is being used concurrently with at least one other standard therapy (examples include an antimalarial such as hydroxychloroquine, systemic corticosteroid such as prednisone, or other immunosuppressants such as azathioprine, mycophenolate mofetil, methotrexate) OR (b) patient is determined to be intolerant to standard therapy due to significant toxicity as determined by the prescriber; and (iv) the medication is prescribed by or in consultation with a rheumatologist, clinical immunologist, nephrologist, neurologist, or dermatologist. OR B) Patient is Currently Receiving Benlysta Subcutaneous or Intravenous: Approve for 1 year if ALL of (i) patient meets ONE of (a) the medication is being used concurrently with at least one other standard therapy OR (b) patient is determined to be intolerant to standard therapy due to significant toxicity as determined by the prescriber; (ii) the medication is prescribed by or in consultation with a rheumatologist, clinical immunologist, nephrologist, neurologist, or dermatologist; and (iii) the patient has responded to Benlysta subcutaneous or intravenous, as determined by the prescriber (examples of response include reduction in flares, reduction in corticosteroid dose, decrease of anti-dsDNA titer, improvement in complement levels, or improvement in specific organ dysfunction).

Prior authorization is required for coverage of Benlysta (belimumab subcutaneous). All approvals are provided for the duration(s) specified in the policy (months counted as 30 days when applicable). Because of the specialized skills required for evaluation, diagnosis, and monitoring, Benlysta must be prescribed by or in consultation with a clinician who specializes in the condition being treated (for example, rheumatologist, nephrologist, clinical immunologist, neurologist, or dermatologist as specified by indication).

Approvals are time-limited per indication (e.g., 6 months for initial lupus nephritis therapy; 4 months for initial SLE therapy; 1 year for continuing therapy when criteria met).
