Coverage Criteria for Fecal Calprotectin Testing

Medically Necessary Indications

Fecal calprotectin is considered medically necessary when EITHER of the following criteria is met:

Distinguish IBS from IBD: For the purpose of distinguishing irritable bowel syndrome (IBS) from inflammatory bowel disease (IBD) in individuals with chronic diarrhea

Use as a noninvasive screen to identify patients who may require endoscopic evaluation; results should be interpreted in clinical context.

Management of IBD: For the management of inflammatory bowel disease

Includes monitoring disease activity, assessing response to therapy, predicting relapse, and postoperative recurrence surveillance; interpret with clinical correlation.

Supported clinical indications

Covered when interpreted in the appropriate clinical context and used for IBD-related indications supported by evidence and professional guidelines.

IBD diagnosis and monitoring: Use of fecal calprotectin to differentiate IBS from IBD in symptomatic patients; to assess response to therapy; to evaluate suspected relapse; to monitor disease activity during maintenance; to predict relapse after mucosal healing; and to detect postoperative recurrence of IBD

Policy statements that previously addressed testing for serological and genetic markers for diagnosis or management of IBD, and statements about therapeutic drug monitoring for IBD, were removed from this policy during the April 15, 2025 annual review. These statements are no longer included in the current policy document.

Use of fecal calprotectin outside the two specified indications (distinguishing IBS from IBD in individuals with chronic diarrhea; and for management of IBD) is not supported by this policy. Testing for other investigated conditions has limited, primarily observational evidence, assays lack FDA approval for those uses, and such applications would be considered not medically necessary or not covered.

Fecal calprotectin testing has been explored in a broad set of non‑IBD conditions (for example, autism spectrum disorder, Parkinson’s disease, and cystic fibrosis), but the literature is largely small, heterogeneous, and non‑experimental. Because no FC assays are FDA‑approved for these indications and the available evidence is limited, the policy considers use of FC for these non‑IBD conditions to be insufficiently supported and therefore not medically necessary.
