Indications for chromoendoscopy: Surveillance for colorectal cancer in individuals at increased risk based on a personal history of inflammatory bowel disease (IBD).

Professional society–based surveillance recommendations

Chromoendoscopy or virtual chromoendoscopy is supported/endorsed by multiple professional societies in specified surveillance contexts when performed with high-definition endoscopy and appropriate biopsy strategy.

Society recommendations: Dye spray chromoendoscopy should be considered in persons with colonic inflammatory bowel disease undergoing surveillance colonoscopy, particularly if a standard-definition endoscope is used or if there is a history of dysplasia; virtual (electronic) chromoendoscopy is a suitable alternative to dye spray chromoendoscopy for dysplasia detection when using high-definition endoscopes. A structured visual assessment using high-definition white light and/or electronic chromoendoscopy with photo documentation is recommended for polyps, and targeted biopsies of concerning lesions are advised; when white-light endoscopy without chromoendoscopy is used, extensive nontargeted biopsies are recommended per society guidance.

Evidence is low or mixed for some indications. For example, studies in individuals with Lynch syndrome (LS) include small or underpowered trials and a meta-analysis that did not show a statistically significant improvement in adenoma detection with chromoendoscopy versus white-light endoscopy (OR 1.17; 95% CI, 0.81–1.70).

Professional guidance notes that chromoendoscopy may be considered for certain populations (eg, LS), but larger prospective randomized trials are needed to clarify its comparative benefit before broad conclusions can be drawn.
